Presentation Summary: There have been many studies that show that physical integration of all needed services increased the utilization of referrals. This workshop will provide an example of a fully integrated community care center. Including primary care, psychiatric care, social services addressing all social determinants of care, peer recovery support and wellness activities. We are now measuring the differences in utilization of referrals and health outcomes including recovery historically and then when offered in a "one stop shop". Although opioid overdose death is decreasing, the disease continues to increase in all populations, with severe complications for individuals, families, and communities.
