Presentation Summary: Conventional methadone induction protocols, developed during the prescription opioid and heroin epidemics, are inadequately designed for the pharmacodynamic complexities of ultra-potent synthetic opioids like fentanyl. Our research directly addresses this critical gap by proposing an evidence-informed, high-dose rapid induction strategy in the outpatient setting that acknowledges the physiological realities of contemporary illicit drug markets.
